Hi Future CDA Family!
Yemaly is a native Venezuelan growing up with two brothers and two sisters, in both urban and farming areas. Academically educated in Maracaibo and immigrating to the US in 1992 she received her Certificate of Naturalization in March of 2017. Yemaly lives in Hillsboro with her husband and has three children ages 32, 30 and 12.
Working for Virginia Garcia Memorial Health Clinic as a medical Team Coordinator, Yemaly continued special programs advocating for individuals and families with varying needs. In 2013, she graduated as a Health Promoter from Providence Medicals Outreach Promotores program.
Yemaly joined Pacific’s Interprofessional Diabetes Clinic (IDC) in 2013 and continues there today. As Patient Care Coordinator, she not only assists the patient by facilitating coordination of care, but also facilitates communication between the patient, healthcare providers, and community services by bridging language and cultural gaps, and addressing challenges specific to the patient’s needs. She is committed to provide patients with the tools for a healthier lifestyle and to improve Latino communities.
She is a Master Trainer certified by Stanford Universities evidence-based Chronic Disease Self-Management Program – Tomando Control de su Salud, and Manejo de Su Diabetes (Known also as Living Well with Chronic Conditions). Six months after the workshop, participants had significant improvements in depression, symptoms of hypoglycemia, communication with physicians, healthy eating, and reading food labels. Classes are highly participative, where mutual support and success build the participants’ confidence in their ability to manage their health and maintain active and fulfilling lives.
Additionally, Yemaly was an advocate for the Covid vaccination program, working as a Supervisor for OHA’s Safe and Strong project for two years. The program provided awareness on the importance of prevention of the disease.
She finished her study in May of 2023 to become Birth Doula (With CEA Doula International, LLC) and is registered with OHA as both a Doula and Community Health Worker.
